As required by Nebraska Schools Rules & Regulations Title 173, a report to the Department, summarizing immunization status of students enrolled in kindergarten or 1st grade, 7th grade, and out-of-state transfer students, is required by November 15 of each year. Completing this online survey satisfies compliance with Title 173.
